Pre-CHIP Program Check List

1)  Register your CHIP Chapter with the Lifestyle Medicine Institute by going to: http://www.chiphealth.com/Login/ (requires signed contract to obtain user name and password).
  • Please see the .pdf attachment from the email called, “CHIP Video Licensing and Web Portal”.
  • (By registering your program, it allows public to look up your program details). See here

2)  Download all 18 videos for free by going to: http://www.chiphealth.com/Login/ (requires signed contract for user name and password).
  • Please see the .pdf attachment from the email called, “CHIP Video Licensing and Web Portal”. Or contact our office and request a USB flash drive for $50. Please don’t wait until the last minute to download the videos unless you want a headache!

3)  Download CHIP Information Session PDF Tool w/ Videos: This interactive PDF tool is to be used during your CHIP Information Session to provide a compelling overview of CHIP and to motivate attendees to register for your class. To download, click here. Ensure you have Adobe Flash XII.

4)  Set information session dates.

5)  Tell public (patients, employees, community, etc.) about your program and invite them to information sessions. Have public sign up for CHIP, once your class is full, order your kits.

6)  Find location to host program, size of venue is based on estimation of people in your program

7)  Find a vendor to perform blood work.

8)  Decide who will perform the biometric screening (blood pressure, weight, height, etc.)

9)  Determine who will provide food samples. (Food samples are required at 7 out of 18 sessions) Could be a cafeteria, self-prepared, or a local restaurant that can be promoted as a pro CHIP.

10)  Pay for video licensing (once per program).
