All the materials needed by the participant in the CHIP (Coronary Health Improvement Project) program. Spouse Carton (for 5 spouses)

Spouse Carton contains materials for five Participant's spouses to be given out by the facilitator of the CHIP program. Each carton contains 5 of each of the following items: CHIP Water Bottle, Pedometer, CHIP Lapel Pin, Medication forms. $100.00

InfoSession Pak

The InfoSession Pak has everything you need to for your CHIP Information Session, including the following: "Introducing CHIP" and "CHIP Clip" promotional DVDs, Marketing CD,  PowerPoint presentation CD with printed guidelines, marketing samples, quiz, and more. $15.00

CHIP Assistant softwareCHIP Assistant software

(For PC only). Easy to use, flexible, conforms to US and international measurement standards. The CHIP Assistant will help you to analyze your HeartScreen data entered before and after and will print out individual as well as group reports in a stratified pattern for the coronary risk factors under study. [ Note: This software is being phased out and replaced by a new online program called "Wellsuite." Call our office for more information. $35.00
